Step 1: Request a referral
If your doctor prescribed an infusible or injectable medication for you, ask them to submit a referral to FlexCare using these forms.
Step 2: We’ll send status updates on your referral
We’ll let you know when your provider submits your referral. When prior authorization is complete, we’ll call to schedule an appointment.
Step 3: Experience the FlexCare difference
Our clinics are comfortable and convenient. We offer private and semi-private infusion suites, as well as evening and weekend hours.
